What is MyChart?

UCI Health MyChart is pleased to offer you personalized and secure on-line access to portions of your medical record. It enables you to securely manage and receive information about your health. With UCI Health MyChart, you can:

  • Schedule, request, reschedule and cancel appointments
  • View upcoming appointments and procedures
  • View letters and notes created by your care team
  • View your Health Summary from the MyChart electronic health record
  • View test results
  • Request prescription renewals
  • Access trusted health information resources
  • Communicate electronically and securely with your medical care team
  • View and pay bills
  • Share portions of your medical record with non-UCI Health providers using “Share Everywhere”

How is MyChart protected?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls his/her password, and the account cannot be accessed without that password. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website. MyChart is fully compliant with federal and state laws pertaining to your privacy.

How do I sign up?

You can sign up for UCI Health MyChart with a unique activation code. This allows you to create your username and password after verifying your date of birth and the last four digits of your social security number.

Where do I find an activation code?

Your activation code is printed on your After Visit Summary (AVS), a document given to you at the end of your outpatient clinic visit. If you were recently hospitalized at UCI Douglas Hospital, your activation code is printed on your Discharge Summary. If you misplaced your activation code, you can contact UCI Health MyChart Customer Support at 1-833-469-2478; you can be sent an activation code via United States Postal Service (USPS) mail, via email or text message.

My activation code does not work, what should I do?

For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. Activation codes sent via text or email will expire within 24 hours. If you have problems with an expired code, you can call our MyChart Customer Support Line at 1-833-469-2478.

Can I get signed up for MyChart without an activation code?

Yes; there are multiple ways to get signed up for UCI Health MyChart. Please ask the front desk staff, the medical assistant, or your healthcare provider to help you with instant MyChart sign up during a clinic visit. Also, with UCI Health MyChart Instant Activation, clinic staff can send a text message or email that allows you to complete your UCI Health MyChart sign up on-the-go.

What kind of test results are available in MyChart?

Many of your UCI Health test results are available for your review in MyChart. Results from outside our organization and old historical results may not be available. Some of the types of results that are available in MyChart, when ordered at UCI Health:

  • Laboratory Tests
  • Radiology Tests
    • X-Ray
    • Ultrasound (US/UTZ)
    • Computerized Axial Tomography (CT)
    • Magnetic Resonance Imaging (MRI)
  • Cardiology Tests
    • Electrocardiogram (EKG/ECG)
    • Echocardiogram (Echo)

When can I see my test results in MyChart?

Your test results are usually released to your MyChart account after your provider has reviewed them. This is generally within 1-4 days. Some tests are released in MyChart immediately if the result is considered within the normal range. Tests that are usually immediately released include: laboratory tests such as complete blood count (CBC), comprehensive metabolic panel (CMP), basic metabolic panel (BMP), and certain other panel tests. Most radiology results are released after 4 days.

If you do not see an expected result and have not yet discussed the test result with your provider, please message your provider. You can do this by logging in to MyChart, and then clicking the link for “Messaging” and “Ask a Question.” If you do not see an expected result but have already discussed the test result with your provider, please contact UCI Medical Records. You can do this through MyChart by clicking the link for “Health” then “Document Center” then “Request Records.”

Why are certain test results not shared electronically via MyChart?

Not all test results are available for online viewing. Some results cannot legally be released electronically. Examples of sensitive tests include: pathology and cytology tests, genetic test results and sexually-transmitted disease tests, including Human Immunodeficiency Virus (HIV) tests. If you are expecting a sensitive test result, please contact your care provider or UCI Health Medical Records.

If some of my health information in MyChart is not correct, what should I do?

The accuracy of your health information is important to us. Your MyChart information comes directly from your electronic medical record at your provider's office. At your next clinic visit, please ask your healthcare provider to correct any inaccurate information. Your health information is reviewed and updated in your electronic medical record each visit.

How do I update my personal information (e.g., home address, e-mail or change my password) in MyChart?

For demographic information (home address, email, phone number, preferred name, gender and emergency contacts), log into MyChart and click on “Profile” and then “Personal Information.” To update your password, log into MyChart and click on “Profile” and then “Security Settings.”

If I send a MyChart message to my provider or his/her nurse, when can I expect a reply?

Most messages receive a reply within 1-3 business days. As such, please note that MyChart should not be used for urgent situations. If your medical situation requires immediate attention, please dial 911 or visit your local emergency department.

Who will reply to my MyChart message?

Your healthcare provider may choose to select an associate, often a nurse or administrative staff member, to help manage and respond to your message. Therefore, use of the UCI Health MyChart secure messaging may not be appropriate if your message includes sensitive information that you would only wish to discuss directly with your provider.

Can I view a family member's health record in MyChart?

Yes; proxy access is available in some situations. Proxy access allows a parent (or guardian) to log into his/her personal MyChart account and then connect to a family member’s account. If interested, complete a Proxy Consent Form and return it to one of our medical facilities to request access to this convenient service. For children ages 0-11, parents/guardians will receive full access to your child’s account. For children ages 12-17, to protect adolescent confidentiality, UCI Health uses a proxy policy compliant with state regulations. Proxies of adolescents are allowed to view certain test results, allergy history, family history and immunizations but cannot view/use the full health summary, medications, medical history, current health issues, messaging, and appointments.

Can I ask questions regarding a family member from my MyChart account?

No; MyChart offers direct access to your personal health record. Communication about another individual from you would be placed inappropriately in your health record. Any communication in your record about another individual could potentially jeopardize medical care for both you and the other patient.

Can my spouse and I share one MyChart account?

No; due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish his/her own MyChart account.

What is Care Everywhere?

In order to provide you with safe, efficient, and coordinated care; UCI Health participates in a health information exchange called Care Everywhere. While receiving care and health services at UCI Health, Care Everywhere enables secure sharing of your medical record with your treating clinicians and other participating care providers outside of UCI Health.

Care Everywhere helps your providers both in-and-out of UCI Health to have access to the most up-to-date information in your medical record. It saves you time by not having to bring copies of your medical record when seeing providers

What is in my UCI Health Care Everywhere patient record?

Your UCI Health Care Everywhere patient record can contain information that would be shared for the purposes of treatment. Examples of information that can be shared through Care Everywhere include:

  • Allergies
  • Immunizations
  • Medications
  • Medical history (past medical, past surgical, family and social history)
  • Clinic and hospital treatment summaries
  • Test results
  • Diagnoses
  • Procedures

UCI Health Care Everywhere will not share psychiatry or psychotherapy information, sensitive notes, or other information that require specific authorization to be released under federal and state law. By having timely access to a more complete, up-to-date, and accurate health record, your care providers can make more informed decisions about your care and may reduce mistakes, especially in an emergency. For example, your treating provider will be able to see updated medications, allergies, and immunizations you received at an outside organization.

Who can see my records in UCI Health’s Care Everywhere?

In an effort to provide the best continuity of care, your care providers will be able to access your records through UCI Health’s Care Everywhere. Your treatment team may also request updates to your record and clinical information from the originating facility.

How is my health information protected?

UCI Health is committed to keeping your records private and secure. Clear and strict federal and state guidelines govern how your health information can be exchanged, viewed, or used. Information that identifies you will not be sold or made available for other purposes. It would only be appropriate for your care providers to view your health information, and only when needed to provide or coordinate your care, make referrals, submit mandatory public health reports (such as your vaccination history), or to provide health care benefits to you.

What does UCI Health’s Care Everywhere do for me as a patient?

UCI Health’s Care Everywhere enables your clinical care team at UCI Health to securely access your record and clinical information at other institutions where you have been previously seen. If you see multiple providers outside of UCI Health who participate in Care Everywhere, they will have the most up-to-date clinical information from your care here at UCI Health. This can be a great benefit for general care but also in emergency situations. This secure health information exchange can speed up the care you receive and reduces the need for duplicate testing.

Are there risks to opting out of UCI Health’s Care Everywhere?

Yes. The goal of UCI Health’s Care Everywhere is to provide your care providers secure access to the best available clinical information about your care. By opting out, your care providers may have less clinical information about you to make clinical decisions.

I have opted out of Care Everywhere at UCI Health, why is my information still available from other healthcare organizations?

As Care Everywhere is a clinical data exchange tool across multiple care organizations, you are required to opt-out of each care organization where you have received care. Each care organization is only able to control the release of your patient data related to your visits and care with that specific organization.

UCI Health, UC San Diego Health, and UC Riverside (UCR) Health use the same electronic medical record system, and therefore participate in the same Care Everywhere health information exchange. Opting out of Care Everywhere at UCI Health will also opt you out of UC San Diego Health and UCR Health Care Everywhere. This means that UCI Health, UC San Diego Health, and UCR Health will stop sharing information about you with the health information exchange. Outside of these three organizations, you will be required to contact every other Epic-participating organization that holds your clinical information and opt-out of their Care Everywhere. Until you opt-out of the outside organizations, your health information may continue to be shared with UCI Health.

I was logged out of MyChart. What happened?

UCI Health aims to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
